For patients that are scheduled for a re-exam on their next visit, you can click on the Patient Progress form to bring in on your scheduled re-exam visit. If you are an existing patient and you have not seen Dr. Hoffer in over 2 months, please click on the Patient Update form and bring it in with you.
Patients with any changes in their address, insurance, or contact information may also fill out the Patient Update Form.
